Learn More About Your First Appointment
For students who are accessing The Glenn Crombie Centre for Accessibility, Counselling and Wellness for the first time in relation to disability-related accommodation, you should bring the following documentation (if available) to your first appointment, so we can plan the supports you’ll need at Cambrian College.
For students with a Learning Disability, Specific Learning Disorder, Mild Intellectual Delay, Autism Spectrum Disorder (ASD), ADHD/ADD:
- All previous psycho-educational assessments
- Assessments must have been completed at an adult age or within the last 5 years
- If you have an older assessment, we can discuss options at your intake appointment for getting a new assessment
- Most recent IEP or IPRC
- High school transcript
- Any accommodation letters (if applicable) provided from other post-secondary institutions
For students with an Acquired Brain Injury
- Completed GCC Medical Form or neurological assessment
For students with a Psychiatric Diagnosis (examples include depression, anxiety, bipolar disorder, etc.)
- Completed GCC Medical Form
For students with Chronic Medical Conditions, Mobility or Physical Limitations, Low vision or Blind
- Completed GCC Medical Form
For students who are Deaf, Deafened or Hard of Hearing
- Completed GCC Medical Form and audiology report
