Documentation for First Appointment - Accessibility

Learn More About Your First Appointment

For students who are accessing The Glenn Crombie Centre for Accessibility, Counselling and Wellness for the first time in relation to disability-related accommodation,  you should bring the following documentation (if available) to your first appointment, so we can plan the supports you’ll need at Cambrian College. 

For students with a Learning Disability, Specific Learning Disorder, Mild Intellectual Delay, Autism Spectrum Disorder (ASD), ADHD/ADD:

  • All previous psycho-educational assessments  
    • Assessments must have been completed at an adult age or within the last 5 years
    • If you have an older assessment, we can discuss options at your intake appointment for getting a new assessment  
  • Most recent IEP or IPRC 
  • High school transcript 
  • Any accommodation letters (if applicable) provided from other post-secondary institutions

For students with an Acquired Brain Injury 

For students with a Psychiatric Diagnosis (examples include depression, anxiety, bipolar disorder, etc.) 

For students with Chronic Medical ConditionsMobility or Physical Limitations, Low vision or Blind 

For students who are Deaf, Deafened or Hard of Hearing
